Our Services

RAC offers support through two core service areas: healthcare consulting for organizations and case management for individuals and families.

Each engagement is tailored to the needs, goals, and context of the client.

Healthcare Consulting

RAC provides strategic and operational consulting to health & community-based organizations seeking to strengthen complex care, care coordination, and person-centered care delivery.

This work is designed to help organizations advance critical initiatives with greater clarity, stronger execution, and practical follow-through — with a sharp focus on successful implementation, sustainability, and meaningful results.

Areas of support may include:

  • Complex care strategy and program development

  • Specialized 3-pronged integrated care (bio-psycho-social) advisory services

  • Care coordination and case management design

  • Clinical workflow and operational improvement

  • CalAIM programming and related implementation support

  • Documentation, training, and process refinement

  • Cross-functional collaboration and systems alignment

  • Audit readiness and operational compliance

  • Education and speaking engagements tailored to organizational needs

Whether the need is project-based support, subject matter expertise consultation on a specific challenge, or broader strategic guidance, RAC offers an approach that is responsive, grounded, and tailored to the realities of the work.

RAC provides case management support for individuals and families facing complex health, care, and life circumstances.

This work is designed to provide experienced guidance, advocacy, and care navigation for those seeking a clearer, more supported path forward — particularly during times of transition, uncertainty, or system complexity.

Case management services are currently offered in California only.

Case Management

Areas of support may include:

  • Assessment of needs and individualized planning

  • Navigation across healthcare and community systems

  • Care coordination and communication support

  • Advocacy with providers, systems, and service partners

  • Guidance during care transitions and difficult decision points

  • Resource identification, referral, and connection

  • Governmental program eligibility determinations and application assistance

  • Family support in understanding available options

  • Organization of next steps, priorities, and follow-through

Case management services are always individualized to the needs, preferences, and priorities of each client and will vary depending on the nature of the situation, goals, and scope of support needed.

Working Together

Scope of Services: RAC begins by understanding the needs, goals, and context of each client or organization. From there, services are scoped based on the situation at hand and may take different forms depending on the type of support needed. Engagements may include: consultation, project-based work, short-term strategic support, ongoing advisory partnership, or individualized navigation and advocacy support.

Rates: Rates are tailored to the scope, complexity, and type of support involved, and are discussed and formalized in a written agreement prior to engagement so clients have a clear sense of what to expect. Depending on the nature of the work, fee structures may include: hourly rates, flat-fee deliverables, short-term service packages, or retainer-based arrangements for ongoing support. When appropriate, customized pricing structures may also be developed.

Ultimately, RAC’s goal is not to offer one-size-fits-all solutions, but to provide support that is practical, responsive, and aligned with what will be most useful in real life.

When RAC May Be a Good Fit

RAC may be a good fit for organizations seeking strategic, person-centered support in complex care and systems work, as well as for individuals and families looking for experienced guidance during challenging care journeys, transitions, or periods of uncertainty.

This work is especially well suited to those who value clear communication, collaboration, integrity, and a more intentional approach to moving important work and care forward.
